The Health and Social Care Data Dictionary is a one-stop shop for health and social care data definitions and standards. The Dictionary is managed by ISD Scotland and as new definitions and standards are agreed they will be published on this site.

All use of and access to pages of this website, the Data Dictionary and NCDDP data standards is subject to the terms and conditions of use and access set out here.

The main sections can be navigated using the menu bar at the top of each page. There are four main sections:

  • Data Dictionary A-Z: NHS and Social Care data definitions. Logos will indicate that further information can be found in one of the other three sections. This section contains generic definitions of all the definitions in the dictionary i.e. from the SMR data standards, the Clinical data standards and Social Care data standards. These pages contain core information about the data standards i.e. definitions, format, field length and codes and values.
  • SMR Datasets: Online manuals for the Scottish Morbidity Record (SMR) datasets. These pages contain definitions, format, field length and codes and values as well as attributes, recording guidance, further information and sub data items.
  • Clinical Datasets: Information on the National Clinical Datasets Development Programme. These pages contain definitions, format, field length and codes and values as well as attributes, recording guidance, further information and sub data items.
